R02: Account Closed
The account existed but has since been closed.

What R02 means
The account number is real and the routing number is right, but the account is no longer open. This hits credit files as often as debit files — a former employee closing a bank account is the classic case.
What to do next
There is nothing to fix in the file itself. Get new account details from the recipient, then send a prenote to confirm them before the next live payment. Re-sending the same entry will just be returned again.
Can this happen on a file from this generator?
Yes. R02 can come back on a credit file, so it is worth guarding against. Verify account details with a zero-dollar prenote before the first live payment, and check the file's structure with the ACH file validator before you upload it to your bank.
